Perfectly Flawed Foundation Executive Director Luke Tomsha says another reason for the Statewide Opioid Action Plan was to try and get more people with lived experience involved. Tomsha says he’s created an online form for those with lived experiences to fill out.
He says he represents many people, but that can only go so far.
He says he put the form together to see who is out so the foundation can form a more diverse group of advocates. You can find the form online here but you will have to log in with a gmail account.
