OSF Healthcare is reopening the Midtown Plaza complex, Midtown Health Center and Granville Clinic on July 10.
Areas of care available at these locations include primary care, prompt care, occupational health and rehabilitation.
Former St. Margaret’s Health patients should request a copy of their medical records from SMH. To do this, patients can visit aboutsmh.org and complete the information forms.
SMH is longer accepting record requests over the phone or email. Due to the volume of requests, records will be received in at least 30 days.
